Emergency Information Form | Note that "Name of Person Authorized to Pick up Child (daily)" is a required field. If the child's parents are the ones who are authorized to pick them up daily, please just write "Parents" in this field. Additionally, please write "NONE" on the back side of the form if the questions do not apply to your child. |

Preschool Supply List
Daily
- Backpack (should be a bag the child can carry themselves and big enough that their lunch box and a few other items can fit inside)
- Water bottle (filled)
- Bento box for lunch (Options here) Please do not send the bento box inside another lunch box. Lunches will be stored in the refrigerator. For any items that don't fit in the bento box, please put your child's name on them.
- Outerwear Weather-appropriate outerwear on days it rains or snows (rain boots and raincoats)
Weekly (In on Mondays, Sent home on Fridays)
- Crib-sized sheet
- Small blanket (Some parents prefer to send in a "nap mat" instead of a sheet and blanket. This is up to you!)
- Optional: Small pillow and/or comfort object for nap time
Supplies to Keep Here (Refill as needed)
- Change of clothes: two complete changes of clothes should always be kept in your child's cubby (shirt, pants/shorts, socks, and underwear). The clothes should be weather-appropriate, so remember to send in new clothes when seasons change.
- Sunscreen
- Bug Spray
- Diapers: pull-ups are also fine to send, but must be the kind with side Velcro. Please send in a sleeve at a time.
- Wipes
- Diaper Cream
- Crocs (Explorers only)
